Who says a procedure is still in force?
In most quality systems a procedure is written, approved, and then it stays. Nobody withdraws it, nobody declares it wrong — the world simply moves on and the document does not. Auditors find this easily, because the question they ask is not "do you have a procedure" but "when did you last review it".
