Is management review a meeting, or a record?

The meeting happens, everyone attends, nobody remembers anything. What makes a review auditable is not the meeting but its three parts: which inputs were examined, which decisions came out, and which actions were raised from those decisions. The third is the one most often skipped, and it is exactly what the auditor follows — because a decision with no action attached is a written intention.
